HOW TO APPLY FOR A DECLARATION OF VALUE FOR DEGREES AWARDED WITHIN THE JURISDICTION OF THE CONSULATE GENERAL OF ITALY IN SAN FRANCISCO
Only degrees awarded within the jurisdiction of the Consulate General of Italy in San Francisco are subject to the procedure described in this document. The consular jurisdiction includes:
- Alaska
- California (except for the following counties which are under the jurisdiction of the Consulate General of Italy in Los Angeles: Imperial Valley, Kern, Los Angeles, Orange, Riverside, Santa Barbara, San Bernardino, San Diego, San Luis Obispo, Ventura)
- Hawaii
- Idaho
- Montana
- Oregon
- Utah
- Washington
- American Territories of Samoa
- American Territory of Guam
- Northern Mariana Islands
- Johnston Atoll, Wake Island, Midway Islands
REQUIRED DOCUMENTATION
The following documentation is required:
- A photocopy of the applicant’s passport signed by the applicant. For non-EU citizens the photocopy must be notarized (copy certification).
- A separate application form must be completed for each degree. The form must be dated, signed and submitted.
- A photocopy of the applicant’s diploma (high school or bachelor’s or master’s or Ph.D.) must be duly acknowledged by a notary public (in Washington state, the acknowledgement by a notary public is not required. In Utah, the acknowledgment is required only for private schools and universities).
- An Apostille for international use, as certified by the Secretary of State. It MUST be physically attached to the acknowledged diploma.
- An original Official Transcript for the diploma that will be submitted. If a Declaration of Value is required for a high school degree, the transcript should include the AP exam score. The transcript must be signed by hand by the Registrar and duly acknowledged by a notary public (in Washington state the acknowledgement by a notary public is not required. In Utah, the acknowledgment is required only for private schools and universities). Please be advised that a photocopy of the Original Transcript is not acceptable.
- An Apostille for international use, as certified by the Secretary of State. It MUST be physically attached to the acknowledged official transcript.
- For continuing studies (laurea, laurea magistrale, master, dottorato di ricerca) only: No fee is due for the issuance of a Declaration of Value.
- For all other purposes, payment may be made via money order, or cashier’s check, or international check in US dollars to the Consulate General of Italy in San Francisco. Please note that no other forms of payment will be accepted. Please see the “FEES” section below.
- For continuing studies only: A photocopy of the pre-enrollment acceptance letter from the Italian school or university.
- One prepaid, pre-addressed envelope (UPS or USPS) must be included in the application bundle. This envelope will be used for the return of documents and declarations of value. FedEx envelopes will not be accepted.

In the event that a Declaration of Value is requested for more than one degree, it will be necessary to complete the same number of application forms accompanied by the requisite supporting documentation. A separate Declaration of Value will be provided for each individual request. The various applications and related documents may be enclosed into a single envelope, which will be returned with the prepaid, pre-addressed envelope provided by the applicant.
In the event that two different academic degrees are recorded in one transcript (e.g. a master’s and a Ph.D.), the respective pages relevant to the first diploma and the set of pages relevant to the second diploma should be separated (if possible) and attached to their respective apostille. In the event that the aforementioned separation is not possible (e.g. due to incomplete information, or when one page refers to both diplomas), please obtain two original official transcripts and be sure that each is attached to their respective Apostille.
DECLARATION OF VALUE RELATED TO Ph.D.
The Ph.D. Transcript must include the following information:
- The methodology used for admission (e.g. interview, open competition, etc.)
- The date of admission
- The duration of the Ph.D. program (in years)
- The date of actual completion of the Ph.D. program
- The date of award
- The methodology (by teaching, by research, etc.)
- The full title of dissertation
- The repository hyperlink pointing to the full dissertation.
Should any of the items 1-8 not be included in the transcript, the applicant must provide supplementary evidence. Evidence of any previous university degree required for the admission to a Ph.D. program must be provided.
FEES

The fees are updated on a quarterly basis, on January 1st, April 1st, June 1st and September 1st, based on the euro-dollar exchange rate.
The fees reflect the cost for one Declaration of Value. In the event that multiple applications are submitted, the fees shall be computed as the sum of the individual fees. Joint payments are accepted.
IMPORTANT WARNINGS FOR ALL APPLICANTS
In order to obtain an Apostille on a document, the school official’s signature must first be authenticated by a notary public. Please note that neither a parent, nor an exchange program coordinator, nor the student themselves may attest that the document is an original or a copy and then have their signature notarized.
The authenticated signature must be that of the school official who signed the document.
Every instance of the applicant’s full name (first, middle, and last) reported on your application(s), diploma(s), transcript(s), translation(s), and Ph.D. dissertation (in any) must coincide exactly with the applicant’s full name on their passport. This applies also to any previous degree and its declaration of value. Any change of name (e.g. by marriage, etc.) must be officially documented.
Applications with inconsistencies in the applicant’s name will be rejected.
