1. A VALID ITALIAN IDENTITY DOCUMENT of the Italian parent (passport or identity card). If the parent is deceased, submit a copy of the last valid Italian identity document held by that parent;
2. CERTIFICATE OF CITIZENSHIP (CERTIFICATO DI CITTADINANZA) issued by the competent Italian Municipality (Comune) no more than six months prior to the date of the request for registration of the birth and expressly stating that the person has possessed Italian citizenship from birth.
