Documentation Proving the Exclusive Possession of Italian Citizenship by the Parent(s)/Grandparent(s)

For cases falling under POINT 2, the following documentation proving the exclusive possession of Italian citizenship by the Italian parent/grandparent must also be submitted:

1. A photocopy of a VALID ITALIAN IDENTITY DOCUMENT of the Italian parent/grandparent (passport or identity card). If the person is deceased, submit a copy of the last valid Italian identity document held by that person;
   2. FULL BIRTH RECORD (ATTO DI NASCITA INTEGRALE) of the Italian parent/grandparent, indicating both paternity and maternity and including any annotations, issued by the competent Italian Municipality (Comune) no more than six months prior to the date of the request for registration of the birth;
   3. HISTORICAL CERTIFICATE OF CITIZENSHIP (CERTIFICATO STORICO DI CITTADINANZA) issued by the competent Italian Municipality (Comune) no more than six months prior to the date of the request for registration; of the birth;
2. HISTORICAL CERTIFICATE OF RESIDENCE (CERTIFICATO STORICO DI RESIDENZA) issued by the competent Italian Municipality (Comune) of the Italian parent’s/grandparent’s last residence or AIRE registration;
   5. Only if the Historical Certificate of Residence shows periods of residence abroad: a CERTIFICATE OF NON-NATURALIZATION issued by the competent authorities of every country in which the Italian parent/grandparent resided. For the United States, the competent authority for the issuance of such certification is U.S. Citizenship and Immigration Services (USCIS);
3. A photocopy of the Green Card or visa.
